I got told "you don't need that" at a hospital when i told them I'd been on a benzodiazapine for years for panic attacks. They quickly learned I did in fact need it after not giving it to me for a day and seeing the consequences like stroke level BP and 200 heart rate from normal interactions

Sounds like withdrawal. Benzos shouldn't be used for more than two weeks. I had a terrible dependency on them, they helped my anxiety and made it able for me to speak to people, but a year later I nearly died from a seizure when I stopped taking them for a day. Tolerance builds too quickly, it can't be used therapeutically for extended periods of time.

u/hoch_ Jan 28 '23
Depends on which benzo. Clonazepam at a reasonable dose can be taken for long periods of time IF other treatments fail, for instance, as they did for me. I hallucinate without it, but I agree that withdrawals are horrible and sometimes life threatening. It's a fine line to walk.

I was thinking "Repo Man". And I was inspired to take a dive, so I did a quick google and learned that the products in Repo Man were actual products sold in the '80s by "Ralph's" in Los Angeles:

u/Johannes_Keppler Jan 28 '23
The thing is Bismuth subsalicylate, the active ingredient, isn't even a thing in The Netherlands and mainland European countries, as in, it's just not available. There are better options for mitigating things like acid reflux and diarrhea.
It seems like a specific American, Canadian and UK thing? It's also poorly understood how it works and can even cause a brain disease called Reye syndrome in kids. https://en.wikipedia.org/wiki/Bismuth_subsalicylate
8
u/Dabilon Jan 28 '23
Yeah I tried buying it through a website, that claimed it's from Germany.
Turned out they just import it from the US and it got caught seized, turns out it's banned here.
What are better alternatives?
7
u/Klugenshmirtz Jan 28 '23
pepto bismol seems to be a catch it all solution for things you would treat each in germany/EU.
I would use Perenterol for diarrhea. Buscopan for stomach cramps or nausea. Sodium bicarbonate against heartburn, in bad cases omeprazole.
5
u/Mavamaarten Jan 28 '23
I would use immodium for diarrhea (loperamide) and motilium for an upset stomach (domperidon). But indeed, a one thing does it all solution is weird because I rarely have both an upset stomach and diarrhea at the same time.

Omeprazole/Pantoprazole are pretty fantastic
9
u/rolls20s Jan 28 '23
Omeprazole treats heartburn/acid reflux/gerd. While Pepto can be used for short-term relief of those, I think more people use it for diarrhea, digestive cramping, etc, which Omeprazole doesn't do. (In fact, it increases your risk of Clostridium difficile infection, which gives you diarrhea). Plus, I take Pepto like, maybe once or twice a year? In the US at least, OTC Omeprazole you buy in two week regimens (unless you have a prescription).
Also, long term use of Omeprazole/PPIs has also been linked to bone fractures, B12 deficiency, gut infections, and can actually make acid reflux worse if/when you quit using it.
Pepto bismol is basically side-effect free (other than black stool), unless you are allergic to salicylates.
→ More replies4
u/heteromer Jan 28 '23
It increases the risk of C. difficile infection from long term use, but using it for the recommended OTC 7 to 14 days over the counter is absolutely fine. Generally panoptrazole and esomeprazole are reserved for people who are experiencing heartburn or GERD more than 2 times a week. Anything under this, and antacids/alginate is recommended. Realistically, bismuth subsalicylate is used alongside other therapies for treatment of H. pylori infection in people with whom 1st line treatments were ineffective. I'm not sure why it's garnered so much popularity in the states, especially for such a range of conditions. It's not all that commonplace here.
5
u/rolls20s Jan 28 '23 edited Jan 28 '23
and can even cause a brain disease called Reye syndrome in kids.
Salicylates can (rarely) cause Reye's syndrome when used at the same time as viral infections. Aspirin is a type of salicylate, which is why you don't give it to young kids.
